چکیده

A 42-year-old male presented to the Emergency Department complaining of right lower quadrant pain of 2 days duration. He had no previous surgical history. On physical exam, the patient was afebrile with diffuse abdominal tenderness and peritoneal irritation. White blood cell, electrolytes, amylase and lipase serum levels were normal. A contrast-enhanced CT scan of the abdomen and pelvis (Picture 1) showed diffuse streaking of the greater omentum extending downwards and anteriorly to reach the pelvis, with a focal mass of fat density showing streaks in a whirling pattern. The fatty mass was located anteriorward in the greater omentum just below the ascending colon. There was no thickening of the adjacent bowel walls or abscess formation. Exploratory laparotomy revealed diffuse infarction of the greater omentum with a focal mass of omental torsion which extended along the lateral abdominal wall terminating in the upper right abdomen. Segmental infarction of the greater omentum is a rare cause of acute abdomen (1). Its etiology is uncertain although several predisposing factors have been underlined such as congenital venous anomalies, sudden change of position and substantial meal (2). The clinical picture simulates an appendicitis, diverticulitis, or cholecystitis (1, 3). Computed tomography scan findings such as a streaking or “whirling” pattern of inflammatory tissue and a fluid cavity based on the degree of necrosis present, can help to make this diagnosis preoperatively (1, 4). This will help to avoid unnecessary surgery since conservative management has been suggested in the absence of complications (1, 3).

Omental Infarction due to Omental Torsion

Omental torsion is a rare cause of acute abdomen and sometimes requires surgery. Recently, we encountered a case of omental torsion diagnosed as omental infarction preoperatively. An 18-year-old male presented to our emergency room with a chief complaint of lower abdominal pain since previous 2 days. Because of his history of Down syndrome, an abdominal examination was very difficult. Plain abd...

Left sided omental torsion with left inguinal hernia: a rare entity.

the omentum and the stranding continued inferiorly into left inguinal hernia sac (Fig. 2). The diagnosis of omental torsion with hernia was confirmed on laparotomy. Omental torsion usually occurs on the right side because of greater mobility of the right omentum. A left-sided omental torsion is rare and is difficult to diagnose clinically. Torsion may be primary, or secondary to underlying caus...

Torsion of the Greater Omentum Secondary to Omental Lymphangioma in a Child: A Case Report

BACKGROUND Omental cyst and omental torsion both are uncommon but important causes of acute abdomen with a difficult clinical diagnosis due to nonspecific features. Here we report a case of an eight year old child with acute abdominal pain referred for USG and CT scan which revealed two cysts in greater omentum leading to secondary omental torsion. CASE REPORT An eight year old male child pre...

Acute Abdomen due to Primary Omental Torsion and Infarction

Background. Torsion of greater omentum is a quite uncommon cause of acute abdomen. It can be primary or secondary but in both cases omentum twists upon itself and causes omental segmentary or diffuse necrosis. Symptoms are unspecific and preoperative diagnosis is difficult. The widespread and increasing use of computer tomography (CT) in differential diagnosis of acute abdomen can be useful for...

Omental infarction: a rare cause of acute abdominal pain.

Omental infarction is an uncommon but important cause of acute abdominal pain, which frequently mimics other surgical presentations. The current series describes five cases of idiopathic omental torsion and infarction and provides a review of the literature. We discuss the diagnosis of primary omental infarction as well as the role of conservative and surgical management.
